Jersey Assistance for Community Caregiving (JACC)

What is JACC?

JACC is a State-funded program that provides a broad array of in-home and community-based services to individuals age 60 and older who meet clinical eligibility for nursing home level of care and who desire to remain in their homes within the community. The JACC program is designed with the goal of diverting or delaying the placement of the individual in a nursing facility. JACC aims to strengthen participants' networks of informal caregivers and to maximize autonomy by providing participants with the opportunity to direct their own care, including hiring their own providers, if desired.

What services does JACC offer?

All JACC participants receive care management services. A plan of care is created for each participant as the result of a collaboration between the participant and his or her care manager. With the participants needs, desires, and goals in mind, the plan of care specifies additional services to be delivered, which may include:

  • Adult day health services
  • Chore services
  • Environmental Accessibility Adaptations (home modifications)
  • Homecare services
  • Home-delivered meal service
  • Personal Emergency Response System (PERS)
  • Respite care
  • Social adult day care
  • Specialized medical equipment and supplies
  • Specialized medical equipment and supplies evaluation
  • Transportation services

Who is eligible for JACC?

An individual is eligible for JACC  when the following criteria are met: 

  • Is a United States citizen or Qualified Alien
  • Is a resident of New Jersey
  • Is 60 years of age or older
  • Resides within the community (not in a facility) 
  • Has been determined to require nursing facility level of care
  • Meets financial eligibility requirements
  • Is not participating in other state-funded programs

Are there limits to JACC services and is there a co-pay?

 Yes. A monthly cost cap is applied to JACC services for each participant. The service package provided is based on an assessment of the participant's needs, unique plan of care, and availability of services and funding.  JACC participants may contribute to the cost of their services. The co-pay obligation is based on the participant's income applied to a sliding scale.

Who provides these services?

Services may be provided by qualified service providers or by qualified participant-employed providers (PEPs). All service providers must demonstrate competence in the service to be provided, and must meet certain qualifications set by the Department of Human Services. The PEP option allows a participant to employ his or her own provider(s) and to direct his or her own care. The ability to direct one's own care will be confirmed prior to the employment of PEPs.

Is JACC a Medicaid program?

No. JACC serves individuals who are not enrolled in a Medicaid program such as NJ FamilyCare or Medicaid Managed Long Term Services and Supports (MLTSS).
